Insurance & Fees
Light Street Psychotherapy is an out-of-network, private-pay provider. We do not contract directly with insurance companies. This model allows us to prioritize your clinical needs and privacy without the restrictions often imposed by managed care.
Out-of-Network Reimbursement
Many PPO insurance plans provide some form of reimbursement for outpatient psychotherapy. While we do not bill insurance companies directly, we provide a monthly Superbill (an itemized statement with necessary diagnostic and billing codes) for you to submit to your carrier.

- Initial Intake: 90791
- Individual Therapy (60 min): 90837 (Modifier 95 for Telehealth)
- Individual Therapy (45 min): 90834 (Modifier 95 for Telehealth)
- Family Therapy: 90847
Questions for Your Provider
To understand your coverage, we recommend calling the member services number on your insurance card and asking:
- Do I have out-of-network benefits for mental health?
- What is my yearly deductible, and how much has been met?
- What is the reimbursement rate (percentage) per session once the deductible is met?
- How do I submit claims for reimbursement?
- Are there any caps on the number of sessions or the “allowable amount” per session?
- Is a referral from a primary care physician required?

Choosing an out-of-network provider offers several clinical advantages:
- Total Confidentiality: Insurance companies cannot audit your records or require treatment plans to authorize care
- Clinical Autonomy: You and your therapist—not an insurance adjuster—determine the type, frequency, and length of your treatment
- No Surprise Bills: You avoid the “after-the-fact” coverage denials that often lead to unexpected balances with in-network care
Payment & Policies
- Payment Methods: Light Street Psychotherapy accepts Cash, Check, Zelle, and Credit Cards (Visa/Mastercard)
- Note: A 3% processing fee applies to all credit card transactions.
- Due Date: Payment is due in full at the time of service
- Cancellation Policy: We require 24 hours’ notice for cancellations. Appointments cancelled with less notice will be charged the full session fee
